Background:

Breast cancer is the most common invasive cancer to affect women in the world. It is the second highest cause of cancer death in women after lung cancer. Improvements in breast cancer detection methods have increased incidence, but mortality has steadily declined. An improved understanding of the epidemiology of breast cancer, including reproductive, genetic, and environmental risk factors, has led to more informed patient counseling and helped guide screening and management practices

 

Recently, automated breast ultrasound (ABUS) systems were developed, and promising results were reported in both screening and diagnostic setting. The current ABUS units, with high-frequency broadband transducers, have been shown to have some advantages over handheld US, namely, reproducibility and reduced operator dependence. However, ABUS systems produce hundreds of images to cover the whole breast, and the images must then be reviewed by radiologists

This is a phase II, prospective randomized controlled study conducted at the National Cancer Institute (NCI), Cairo University.

Between November 2019 to November 2021, we enrolled 139 patients meeting the inclusion the criteria, who were diagnosed as clinically Node positive breast cancer disease, then received neo-adjuvant chemotherapy and underwent surgery (mastectomy or conservative surgery) and turned to have ypN0. They were randomized into the 2 arms as follow;

  • ARM 1 (No LNI): 69 patients didn’t receive regional nodal irradiation.
  • ARM 2 (LNI): 70 patients received regional nodal irradiation.

 

After median follow up of 40.69 months, the 3-year LRR was 97% for No LNI and was 97.9% for LNI (p=0.570), as well as the 3-year DFS was 86.6% for No LNI and was 87.6% for LNI (p=0.811) and the 3-year OS was 96.5% for No LNI and was 90.7% for LNI (p=0.327).

Introduction: It would be of great clinical benefit if a reliable non-invasive method of determining ALN status in cases of breast cancer was available. PEM is a novel non-invasive diagnostic tool showing promising results for accurate lymph node assessment, having the advantages of being little affected by breast and metal implants, menopausal status, and show almost no influence from previous hormone therapy Patient and methods: 97 female with breast masses were submitted for PEM for axillary lymph nodes assessment   in ross sectional , diagnostic accuracy study.Results: the PEM results showed sensitivty about 83.3 % , specficifty 98.8 % , dignostic accuracy 91.4 % Conclusion: Preliminary study of 97 patients shows promising results for accurate lymph node assessment by PEM, showing shows improved lesion detectability with 83.3% sensitivity 98.8% and sensitivity in assessment of ALN status in patients with breast cancer.

Abstract


This thesis investigates the role of multi-parametric MRI in the restaging and assessment of response in patients with locally advanced rectal cancer following neoadjuvant therapy. The study focuses on using both morphological and functional MRI criteria to propose MRI-based tumor regression grading (MR TRG) and compares these findings with the final pathological reports post-surgery. By evaluating the reliability of MRI in predicting tumor response and restaging, this research aims to enhance the accuracy of non-invasive imaging techniques in guiding therapeutic decisions and potentially improving patient outcomes. The results demonstrate the potential of MRI as a robust tool for assessing therapeutic response, which could lead to more personalized and less invasive treatment strategies for rectal cancer patients.
